A Coast Guard physical is the medical examination and paperwork (Form CG-719K or CG-719K/E) required by the U.S. Coast Guard’s National Maritime Center (NMC). It verifies you meet the medical, vision, hearing, and physical ability standards to hold a Merchant Mariner Credential (MMC) and/or STCW endorsements. The exam must be performed by a U.S.-licensed MD, DO, PA, or NP.
Why it matters: Your Medical Certificate is the Coast Guard’s proof that you’re fit for duty and safe at sea.
Anyone applying for or renewing an MMC (including mariners on domestic voyages) must maintain a current Medical Certificate.
Mariners on vessels subject to STCW must have a valid Medical Certificate.

We follow the Coast Guard’s Merchant Mariner Medical Manual and related rules:
Vision- Deck: at least one eye correctable to 20/40 (uncorrected not worse than 20/200); STCW deck requires 20/40 in each eye (corrected). Engineering & certain other roles: at least one eye correctable to 20/50 (uncorrected not worse than 20/200). Color vision (red/green/blue/yellow) via accepted tests (Ishihara, Farnsworth Lantern, etc.)
Hearing- Audiometry typically averaged at 500/1000/2000/3000 Hz with unaided threshold ≤30 dB in the better ear; functional speech discrimination ≥90% (original) or ≥80% (renewal) at 65 dB.
General medical & physical ability- Review of medical history, medications, vitals, and focused exam.
If indicated, a demonstration of physical ability (e.g., climbing ladders, stepping over 24" coamings, moving through restricted openings) per Table 6-1 guidelines.
Avoid Substances: Refrain from alcohol and non-prescribed drugs, and inform the examiner of all medications you're taking.
Note: The exam and the CG-719K/K-E form must be completed within 12 months of when you submit to the Coast Guard.

1. We’ll complete the practitioner sections of CG-719K/K-E for you.
2. You submit your application to the NMC (email MEDAIP@uscg.mil) as one PDF with any supporting medical information. (Tip: drug-test results go with your MMC application—not the medical form.)
3. The NMC reviews and issues your Medical Certificate with the correct validity dates (STCW up to 2 years; national up to 5 years; pilots up to 5).
