What is a Coast Guard physical?

A Coast Guard physical is the medical examination and paperwork (Form CG-719K or CG-719K/E) required by the U.S. Coast Guard’s National Maritime Center (NMC). It verifies you meet the medical, vision, hearing, and physical ability standards to hold a Merchant Mariner Credential (MMC) and/or STCW endorsements. The exam must be performed by a U.S.-licensed MD, DO, PA, or NP.

Why it matters: Your Medical Certificate is the Coast Guard’s proof that you’re fit for duty and safe at sea.

Who needs a Coast Guard physical?

Anyone applying for or renewing an MMC (including mariners on domestic voyages) must maintain a current Medical Certificate.

Mariners on vessels subject to STCW must have a valid Medical Certificate.

Engineering & certain other roles: at least one eye correctable to 20/50 (uncorrected not worse than 20/200). Color vision (red/green/blue/yellow) via accepted tests (Ishihara, Farnsworth Lantern, etc.)

Hearing- Audiometry typically averaged at 500/1000/2000/3000 Hz with unaided threshold ≤30 dB in the better ear; functional speech discrimination ≥90% (original) or ≥80% (renewal) at 65 dB.

General medical & physical ability- Review of medical history, medications, vitals, and focused exam.

If indicated, a demonstration of physical ability (e.g., climbing ladders, stepping over 24" coamings, moving through restricted openings) per Table 6-1 guidelines.

Avoid Substances: Refrain from alcohol and non-prescribed drugs, and inform the examiner of all medications you're taking.

Note: The exam and the CG-719K/K-E form must be completed within 12 months of when you submit to the Coast Guard.

Submission Methods

Online submission:

  • Photo ID (for clinic check-in).

  • Your filled-out sections of Form CG-719K (or CG-719K/E for entry-level).

  • Current eyeglasses/contacts

  • Hearing aids, if used (with spare batteries).

  • Complete list of medications & dosages (including OTC/supplements).

  • Recent specialist notes for any ongoing conditions (e.g., cardiology, diabetes, sleep apnea/CPAP compliance), prior waiver letters, and previous Medical Certificate/MMC if renewing

What happens after the exam:

1. We’ll complete the practitioner sections of CG-719K/K-E for you.

2. You submit your application to the NMC (email MEDAIP@uscg.mil) as one PDF with any supporting medical information. (Tip: drug-test results go with your MMC application—not the medical form.)

3. The NMC reviews and issues your Medical Certificate with the correct validity dates (STCW up to 2 years; national up to 5 years; pilots up to 5).
